Most importantly, however, is your Survivorship Care Plan — a living document you can share with your healthcare professionals throughout the rest of your life. Detailing and summarizing your cancer, treatments received, medications taken, etc., the plan is an important resource in both near- and long-term recovery as it lays out an after-treatment regimen, instructions on follow-up visits, the need for regular scans, information to help reduce the risk of cancer recurrence, and outlines patient responsibilities moving forward.

Reviewed with you by a licensed professional after treatment, this is the first of many celebratory steps to come as you transition into the survivorship phase of cancer care.
